Hormones are chemicals that coordinate different functions in your body by carrying messages through your blood to various organs, skin, muscles and other tissues. A gland is an organ that makes one or more substances, such as hormones, digestive juices, sweat or tears. It’s a part of your endocrine system and is in charge of making several essential hormones. Even with an abnormally low level that is replicated on a repeat test, the decision to begin testosterone replacement therapy and the proper dose requires a careful conversation with your doctor. Although testosterone may make prostate cancer grow, it is not clear that testosterone treatment actually causes cancer.
Testosterone production, specifically in men, hinges predominantly on LH. It influences mood, energy levels, and overall well-being. It is appropriate to entertain the possibility of additional endocrine dysfunction in any man presenting with hypogonadism. However, given the expense of MRI and the fact that the vast majority of hypogonadotrophic hypogonadism cases initially assumed to be idiopathic turn out not to have any structural lesion, it is not uniformly recommended to obtain imaging unless one of the above situations is observed or if it is expected to affect management. As with other recommendations in medicine, the decision to obtain pituitary imaging should be based on the likelihood of the result significantly contributing to clinical management. While evaluating for central hypothyroidism or (much more rarely) central hyperthyroidism, a serum free thyroxine (T4) or free thyroxine index (FTI) must be done concomitantly with thyroid stimulating hormone (TSH) measurement, as the TSH alone can be misleading (52,53). Screening for adrenal insufficiency can begin with an early morning (no later than 9 AM) blood draw for cortisol and ACTH.

But higher-than-normal FSH levels are usually a sign or side effect of hypogonadism when an issue originates in the gonads. Hypogonadism occurs when sex glands, or gonads (either testes or ovaries), produce few sex hormones, if any. Except for abnormally high levels of FSH in kids, which can cause early puberty, abnormal levels of FSH — whether too high or too low — usually point to hypogonadism. If the adenoma is of a certain size or is growing, it can put pressure on your pituitary gland or block blood flow to it. For children, higher levels of FSH and LH than expected based on age — in addition to the development of secondary sexual characteristics — are an indication of precocious (early) puberty. Most often, higher-than-normal levels of follicle-stimulating hormone (FSH) are a sign of an issue in the ovaries or testes (gonads).
For individuals undergoing certain types of hormone replacement therapies, LH levels can be monitored to gauge the effectiveness of treatment and ensure proper dosing. A healthcare provider will typically order a blood test to measure LH levels for several specific reasons. This GnRH travels a short distance to the pituitary gland, instructing it to produce and secrete both LH and FSH into the bloodstream. It affects male fetuses as they develop in the uterus, as well as teenage sexual development during puberty.
Several studies have also suggested elevation of prolactin may detrimentally affect testosterone metabolism by 5-alpha reductase to the more potent androgen DHT (37,38). Prolactin receptors have been shown to be present on Leydig and Sertoli cells in mammalian testes, where physiologic levels of prolactin increase the number of LH and FSH receptors, among other effects (32). The syndrome of hyperprolactinemia in men is more polymorphic and tends to be recognized later in life than in women (31). Gonadotrophin levels and spermatogenesis will be variable, based on the predominant primary versus secondary hypogonadal effect of the insult. Importantly, several conditions can lead to a combined primary and secondary hypogonadism in men, including glucocorticoid use, alcohol abuse, hemochromatosis, sickle cell disease, granulomatous disease such as sarcoidosis, and thalassemia (17,27). This phenomenon has been confirmed by others, and translated into therapeutic developments for both male and female infertility (22,23). Investigation of other pituitary axes and the need for pituitary imaging is based on clinical suspicion and results of initial evaluation.

However, the rise in LH in aging men is usually less dramatic than the rise in FSH and LH seen in menopausal women.
Lower-than-normal FSH levels due to an issue with your hypothalamus or pituitary gland directly cause hypogonadism. These sex hormones, in turn, feed back to the hypothalamus and pituitary, regulating their own production. Since LH production is governed by the pituitary gland, which in turn is controlled by the hypothalamus, abnormal LH levels can signal a problem within this master control system. The gonads then release sex hormones (estrogen, progesterone, testosterone) back into the blood. Lower-than-normal testosterone levels typically only cause symptoms in males.
